Health and Local Government Partnerships are emerging as an essential part of the new drive to deliver better services to patients at home, in the community and in hospital. In the biggest programme of reform ever implemented in the NHS, cooperation between health and social services is making it possible to implement a variety of partnership models, such as integrated home care teams and rapid response teams for older people.
